Form 4 Firearm Trust

When submitting the Form 4, it is my understanding that a copy of the "complete" Trust should be enclosed along with the $200.00.

Should the Trust copy be notarized/certified or will just a photo copy suffice?

Does a Certificate of Compliance need to be completed by the Trustee signing the the Form 4?

I appreciate any direction you guys can offer because I have already received some yes's, no's and maybe's.

You only notarize the original.

A copy of the original notarized trust will suffice.

Yes the trustee will complete the Certificate of Compliance on behalf of the trust.
