I am also curious to hear how this happened. Doesn't seem possible if it was installed properly...

That could be, it also resulted in a baffle strike. Gun was a PSA premium upper and was firing XM193J. We also had a Specwar K go ballistic last year from a baffle strike that still (no pun intended) baffles me to this day.

I dig the ASR for sure but I do think the Keymount is a little better. It is sweet to be able to use the Omega on the gun the Sandman will go on. Though I don't think I will be swapping every ASR muzzle device for a keymount device. I have had some issues with my original first run Omega, the threads bottoming out and the lock teeth don't line up quite right. I still need to send it back, SiCo fixed the problem early on, it just needs the ASR mount swapped for the revised one.
I have wondered about the possibility of nicking the threads on an ASR muzzle device manifesting possible alignment issues but have not seen or heard of this happening.
It is just cool to get and increase in compatibility, its a two way street, but probably SiCo will benefit from Omega sales to Sandman owners most.

$249 is MSRP. You can order one right now for $167! Excellent price for what it allows you to do.

This is literally the 6th one I have heard about this year. I own an omega and I can't see how this keeps happening. My best guess is it's a popular all in one can so a lot of people are picking them up that have never owned a can before and it user error or under the same premise are using crappy upper/barrels and are having issues that way. I am also baffled.

Of course this is announced literally the day I got my barrel back from the gunsmith with a saker flash hider pinned. I have a saker 556 and a sandman-s, so being able to use both with the same devices (especially the superior keymount system) is very appealing. I plan on getting this down the road!

I would just sell the barrel and get a new one. Bad timing indeed. I would never have thought they would release something like this.

Exile will likely drop ship from RSR in Texas. My 3 Lug I just ordered last week came from RSR directly.
