I did not completely remove retaining pin.

The first two times I cleaned it, I had no problems. I ran more rounds through it this time getting it more dirty...
Around how many rounds would you say you put through it?
    Where they reloads?

No reloads, surplus and match ammo.

Probably about 400 rounds. Doing mostly precision shooting. Not much running and gunning high volume like with 5.56.

About 150 rounds since last cleaning.

Only 150 rounds and you where having to use a punch to get it out. That seems rather odd to me. Did you send it to LMT to have them take a look to see if there is anything that needs changed in the design and for a replacement?

No. We did not take it all the way out. We couldn't even get it to move out far enough to remove the firing pin.

So I took it apart (captive pin and roll pin) and we discovered that the actually retaining pin somehow got bent. I did a temp fix by straightening it, but it will need replacing.
