I finally built up my AXTS A-DAC-F lower and hit the range today. I noticed with USGI mags (Okay Industry mags purchased new in the last 6 months), the mags would occasionally drop out while I was shooting. I'm certain they were locked in as I'd give it a good tug and then after a few rounds they'd just drop out. I tried tightening the mag catch but it did not help.
Got home and figured out if I press the mag against side of the magwell opposite to the mag catch and pull down I can yank the mags out after insuring they're properly seated. I've tried a few different mag catch springs (the AXTS mag catch spring is proprietary) however can't seem to get my USGI mags to lock in there.
Could this be an issue with the mag catch, too large of a mag well, or magazine related? I tried multiple USGI mags (Okay, D&H) and they all seem to have this problem. Mag catch used was from a new Daniel Defense LPK.
For what its worth, my first AXTS A-DAC-F lower would not drop PMAGs freely, I sent it back and after inspecting it and concluding it could not be fixed they sent me a new lower (the current one).

I have since determined that it was indeed the mag catch that was causing the issue
I went down to the range today and after asking a few guys (you don't know how akward it is to ask someone if they're willing to let you quickly borrow their mag catch until you do it ) I finally found a guy that was willing to help me troubleshoot. The guy had a DEL-TON rifle, but the mag catch worked great and eliminated my issue.
I'll be ordering a new mag catch now - thanks for the help fellas.
