f. Photographs and Fingerprints. An individual transferee, except if licensed as a manufacturer, importer, or dealer under the GCA, must (1) attach to Item 16, a 2 inch x 2 inch photograph of the frontal view of the transferee taken within 1 year prior to the date of the application and (2) attach two properly completed FBI Forms FD-258 (Fingerprint Card with blue lines) to the application. The fingerprints must be clear for accurate classification and taken by someone properly equipped to take them.

What do you put for "Reason for being fingerprinted" on the fingerprint cards?

Really random question, I bought (paid for, still at the dealer)a Gemtech can for my one AR today, got all of the paperwork (going to see police chief tomorrow), is the fingerprint card the same as the one I got with my arizona non-resident CCW?

No. You must use the NFA Branch fingerprint cards.

I read through the instructions on the form 4, section 2f says I need two fingerprint cards, am I reading that right?

Yes. Two fingerprint cards with the ORI for NFA.

I tried to get cards from ATF but couldn't get their online order form to work. I used blank cards from my SO and printed labels with the correct ORI on them and affixed them to the fingerprint cards. Two days later.....my ATF fingerprint cards came in the mail. :/ I hope they don't kick my cards back cause I'll be downrange in two weeks.

Got my local CLEO to do my prints today and sign off on my paperwork. Was no problem, no bs, did it while I waited just asked me to schedule an appointment next time. Was funny, first thing he said to me "you one of those retards paying insane prices for an AR now??" haha, that made my day!

My LGS is still trying to tell me I only needed one fingerprint card though.
