Not for a can, no.
It's not required by law.
But if I'm doing a 5320.20 form for one of my SBRs that I use with a can. I always list the can as well.

Filling out one of these for a suppressor can save you headache down the road.
You should have one of these filled out for all your NFA firearms you frequently might travel with. It should be good for the entire year and keep getting a new one every year.
Do this instead of filling one out for a specific vacation for a week trip.

I fill it out starting at the date which I send it in.
It is good for one years time. It does not have to be a calendar year.
It can go from July 14, 2009 to July 13, 2010 or some other random date.
For your location, just put a hotel that you will stay at.
I also put a condominium that I own.
The location you put does not mean you have to be there or go there at any particular moment in time.
I go to Wyoming, Colorado, South Dakota, and Montana every year. When I leave and when I get back cannot be determined so I just make one for a full years time.
