At room-length distance, wouldn't almost any expanding bullet be pretty effective?

I am no expert, but it seems to me that while some 223/5.56 rounds are better than others, I don't think any are what could be called ineffective.

No, it's clearance between parts that gives the debris a place to go. Looser tolerances will actually result in less reliable firearms because clearances will vary too much between minimum and maximum.
