Form 4 questions for 1st NFA purchase - SBS

I'm a current LEO purchasing my first NFA weapon, a 14 inch barrel Mossberg 590A1, from a fellow LEO in my agency. I'm about to send my completed paperwork in and I have a few questions. I had our permits officer print me on the blue FBI cards (form FD-258), but our agency's cards have a preprinted ORI for the state police and under 'reason fingerprinted' it is preprinted 'pistol permit.' My questions are: 1) can I whiteout these 2 areas since the card is obviously not going to the state police for a pistol permit? 2) what should I put in the ORI portion? 3) what should I put in the 'reason fingerprinted' portion? Thanks in advance for any advice.

My experience is, the NFA branch will not accept fingerprint cards that are not pre-stamped by them(BATF). You should be able to pick some up at a local ATF office.
Under reason, just write NFA.

You can also get the ATF to mail you their fingerprint cards for free through their website. That's the route I took.

ATF will accept them once you insert the proper info. Use a white label for a manila folder/addressing an envelope to cover the incorrect info.

Reason for printing: NFA Transfer

ETA: I would think your agency would have some blank cards as they have to stamp their info in them.

I called the regional ATF office (we have a satellite office with 1 or 2 agents- they referred me along). I had the cards the day after next.

AZDoug hit the nail on the head - you need the WVA ORI. Just because the cards came from ATF doesn't mean they have the proper ORI.

Thanks all for your assistance! I was able to get the correct ORI and address from another source, I just used whiteout and printed the correct info for BATFE in the appropriate boxes. Hopefully that will fly...it's been about 3-4 weeks and I haven't gotten them back, so that's gotta be good.
