I received a Maintenance Action Message today. TACOM has issued a message in regards to the gas key on the M4 and M16A2/4. Has anyone heard of this issue or experienced problems in issue Weapons?

Here is the issue:

2. Issue:

Notifications received from the field on M16 / M4 Bolt Carrier Assembly
deficiencies; reference NSN 1005-01-441-1619. CAGE code 1B1B6.

Testing has verified the following issues:

a. The bolt carrier key does not have the correct inside taper for
the gas tube to correctly align with the key causing reduced gas pressure
and damaging both the bolt key and gas tube.

b. The inner diameter of the front of the bolt carrier prevents
proper bolt installation. As a result, bolt gas ring damage may occur.

c. The bolt carrier key height does not provide adequate clearance,
preventing proper cam pin installation.

Here are the user actions:

3. User Actions:

a. TM 9-1005-319-23&P, Perform WP 008-16 Preventative Maintenance Checks
and Services (PMCS) for the Bolt and Carrier assembly. Also perform WP
0011-1 thru WP 0011-8, (Disassembly, Cleaning, Inspection-Acceptance and
Rejection Criteria, Repair or Replacement, Assy). Pay particular attention
to work packages 0011-6 (Checking for Fit of Bolt Assembly) and (Repair of
Carrier Key), Special tool WP 0039-58, machine key, NSN: 5315-01-310-0370).

b. This should be accomplished at least on a quarterly schedule. This
inspection of your bolt and carrier assembly will allow you to verify that
you have a completely serviceable weapon. Perform
functional check IAW TM 9-1005-319-10 WP 0008-00-1.

That CAGE code belongs to Toolcraft.

I wonder if someone input the wrong cage. It has been known to happen.

I have five of them in my shop. The keys seem to be fine but the bolts are real tight in four of them and I cannot get a bolt in the fifth at all. They are all Toolcraft.

No. They make carriers on contract. I can't remember for whom. I want to say FN.
