Form 5330.20 Certification of Compliance as Trust or myself?

I am getting ready to send of my form 1 ('s) along with the 5330.20..

The SBR will be for my Trust.. do I fill out 5330.20 as the Trust? or as myself?

The 5330.20 asks:

1. Name under which the application was filed. (Last Name, First Name, Middle Initial - or - Name of Corporation, Partnership, or Association, if applicable)

and

2. Name of person certifying to citizenship. (Last Name, First Name, Middle Initial)

are these me personally and should list my given name (First Last and MI)? or just list the name of my trust as: XYZ Trust ?

For 1 - name of the trust.
For 2 - your name

Sign at the bottom in your name. Yes, this is a silly form.
