Has this happened to anyone else?

I just purchased a S&W M&P15A. Fresh out of the box when I released the bolt forward/dry fired the weapon the bolt locked into place in the forward position. After some fiddling with it I was able to get the bolt unstuck but the same thing happened later. I sent the gun back to S&W but was just courious what may have caused this?

Never had it happen (don't own any S&W products)
All guns are test fired/cleaned before they leave the factory and you should not have any issues like that.
But with that knowledge I would have field stripped & cleaned it before doing anything with it as humans do make mistakes and something may not have been installed correctly.

I think you did right by returning it especially if you plan to use it as a duty weapon.
