Safariland triple open top duty magazine pouch, basketweave. This fits M&P .45 magazines, as well as Glock 17/22 9mm, .40S&W, .357Sig etc. It will probably fit M&P 9mm and .40 magazines too, but I don't have any to try out.
I have another one of these on my duty rig that is labeled Glock 17/22 and they are the exact same size, so it will definitely fit both kinds of magazines.
