Certificate of trust?

I feel like an idiot. Is the cert of trust the same as "Certification of Grantor"? I have this on the last page of my quicken willmaker trust paperwork. Below that is the certificate of acknowledgment of Notary Public. Do I just have to sign the Cert of grantor and get the notary to fill out the rest( sign it also in front of the notary) and include my schedule? Thanks

ATF will no longer accept a Certification of Trust. You must send a copy of the notarized complete Declaration of Trust and the Schedule A. You may redact anything on the Schedule A other than NFA items.

Do NOT send the original Declaration. You will no receive it back from ATF.
