Affecting mostly T-Mobile's customers, but also affecting some AT&T, Verizon and their subs. Big cities like L.A., Houston, Chicago, Atlanta and other large metro areas.

Technical issues or malicious act? If malicious, is it foreign actors or internal?

How can the idiot masses be told what to think if their phones don't work? It'd be in the best interest of our enemies to keep the propaganda stream flowing.

Around here it was TMobile calling. Everything else worked fine and all other companies worked fine. Also, all phones worked if you used another phone app regardless of company. So basically you couldn't make a call on a TMobile phone unless you used a different app

Downdetector was where I found out that it wasn't just my phone. I missed many texts and phone calls, people started emailing me when they did not hear back from me.

It was T-Mobile and it seems that those AT&T and Verizon reports were due to them trying to call/text T-Mobile customers and it not working and the error responses not being obvious at who was at fault. I know when I was getting error responses trying to call a contractor it was not obvious at all who was giving me the response, my carrier or his. (I later talked to him and he is on T-Mobile as well so after the fact I learned that we both had the issue).
