Smith Vortex is advertised as self tightening (as are some other muzzle devices). Everyone pretty much agrees with this as the twist of the prongs lends credence.

Here's video proof though, found while looking for something else (barrel whip caused by asymetrical muzzle devices).

I have never had a properly tightened muzzle device come off...

No, but we got dudes out here building AR's with needle nose pliers and chunks of 2X4 so it wouldn't surprise me to know some people are running their muzzle devices hand tight.
