Fingerprint cards

Hi guys.... Hope some of you can help me with these cards...most of it is pretty simple, however, a couple of things seem vague... and I am trying to eliminate any delays if possible..

On the left side of the card below signature of official taking prints, it wants employer name and address, is this for me or the department taking the prints? and, what would be the generic reason for being fingerprinted? Thanks for any help..

reason = NFA purchase

employer address is that of the person who finger printed you.

You do not have to give reason - it's a NFA fingerprint card.
I only wrote reason on one set of NFA cards ( I have a lot more the one class 3 stamps ) so don't worry about it.
