Question: Do I have to remove the USGI FSB when installing a CASV-M or do I have to remove, shave it down or install a gas block, handguard cap, etc or can I just leave the regular front sight base, and install the CASV-M behind it?

The CASV works with your standard barrel nut and delta ring. If you have a carbine length gas system (ala M4) you will have to shave your FSB or replace it with a low profile gas block and remove the hand guard cap for it to work with a CASV-M. The regular CASV will just drop in to a standard M4. If you have a midlength the CASV-M will just drop in.
