Would someone who made an SBR do me a huge favor?

I am a little confused on how to complete the form 4. I'd REALLY appreciate it if somebody who has done a Form 4 for a Trust, could email me a scan of the two pages. Just black out your seriel number, address etc.....

Once my Form 4 is approved, I am going to scan it and block out the above items and I will leave it posted and a link to it here to help others with the same questions!

if u are buying the sbr from a class 3 dealer, they will supply u with the form 4s
in duplicate. if its going on a trust be sure to tell the dealer to put the name of the trust in the transferee block. On the back of the form 4 under the transferee cert be sure to put your full name and sign it. Send a notorized copy of your trust document with your forms along with the $200 check made out to BATFE. Also don't forget to send in a certificate of compliance.

Should'nt take no more than 18 to 30 buisness days for a trust transfer, depending on the examiner u get.
