This was a reload from a locked open bolt using the bolt release.

The mag released the round early, did you tap bottom to seat the mag?

That was happening to me 50 out of 75-100 trigger pulls. Then Steve at ADCO told me to give WOA a call for new buffer springs and buffers.

Problem went away.

The only time I've seen a round get up above the bolt was when a kid shooting an AR15 had a failure to eject and didn't drop the mag or clear the spent case and furiously cycled the weapon until a live round fed off the mag and got wedged up in the upper receiver.

Nope, push mag in till locks then pull on it to make sure it is indeed locked in.

Yup. I've divorced a few mags lately.

Wow. That is completely above the bolt. Didn't think they could quite fit all the way up there. Worst I had happen was a crappy no-name plastic snap cap type deal do that. What a PITA.

Pound that bastard flat. It isn't worth the aggravation it causes, especially if he ends up replacing the gas tube (can't tell from the pic, but it looks probable)

Nah. Put one of those single-shot followers and use it for zeroing (one shot loaded at a time). :)
