I will be putting one of these on a 14.5 bcm middy...hopefully. Will the handguard cap on the fsb need to be removed.

Negative and I personally think it leaves an unsightly gap.

It doesn't need to go and it leaves your options open if you decide to go back with the standard stock setup. I have heard of a few rattling and that may annoy you. If it's solid I would leave it but it is not necessary........
