I have a MP9 and it shoots dead on 0-10 yds about 1-2 inch high at 15 and about 4" high at 25 yds.

Does your MP 9 do the same?

My 1911 .45 is is almost POA/POI from 0-25.

My oldest M&P shot like that. I put a .180" (I think) front sight on and it hasn't been a problem since.

My M&P 40c exhibits exactly as you describe. Dead on at 5-7, a little high left at 10, out to steel plates at 15-25 my POA is literally 4"-6" low right to put on target. Now I get that I need to drift the rear but could never figure out the elevation issue (could very well be my admitted inexperience) but have no such issues with any of my other handguns. Mine are stock sights.

What load are you running?

147 gr loads will shoot higher than 115 gr or 124 gr, typically, if the lighter loads aren't downloaded.

My M&P 9mm FS (4.25") running a 124 gr load at 1,100 fps (+/- 15 fps) at 69 ft above sea level shoots low at <30 ft. I have to drive to the dot using factory sights to hit POA from a rest.

At 25 yards, it's POA/POI using the top of the front sight.

However, I've read shooting high @ 25 yards with the 5" guns seems to be a somewhat regular occurance.

Mine shoots POA at 25. Like was mentioned previously, I've heard of some older models shooting high at 25.

what causes a pistol to shoot high? Bad barrel? its pretty accurate but high.

Bullet trajectory vs front sight height vs POA vs sight picture.

I note that if I align the 3-dots on my M&P stock sight setup, it shoots dead on. If I align the top of the front sight blade with the top of the rear sight blade, it shoots high.

Beyond that, consistent POI different from POA might also be technique.

Here's a chart Louis Awerbuck drew up and hands out at his pistol classes. I took a course from him a couple of weeks ago and saw a few guys significantly change their POI from making changes detailed on the chart.

Some M&P 9mm pistols are documented to have a problem with premature unlocking, which causes the pistol to shoot high at 25 yards. I understand this is particularly noticeable with weaker 9mm ammunition (as opposed to +P or +P+ loads).
