Having trained ~1000 LEOs and EMS/Fire in TECC and TCCC over the last 2.5 years, I've seen it time and time again with students where they get slowed down having to do simple tasks like open a pouch because they didn't think it through.
The easiest solution is with a 2-mag M4 pouch and an IFAK insert. Companies like ATS Tactical, Tactical Tailor, etc. make good inserts that are easy to organize and carry the essentials. You can get surplus Eagle Ind mag pouches for ~$15 on eBay all day long.
It's also probably easier to mount the mag pouch upside down on the cummerbund so that it opens down...makes retrieving self aid equipment easier. I carried smoke grenades that way for years for that reason, and I have never spilled kit out while doing so.
The big Army issued TQ pouch is good. Heed ironman's advice about a closed pouch for TQ storage.
This is the tq holder I got. Is this what you're talking about? The tq is still in the packaging in there. I asked the guy that put on the "stop the bleed" course I took. He said leave it in plastic, it'll add a layer of protection from elements
