Sure enough the ejector rod was loose- once screwed in it functioned as normal. Only thing is it keeps unscrewing- is there a suggested way to tighten it without damaging anything?

There's probably some funky tool for that but I'm not aware of it. You could call S&W tech support. Maybe a dab of 242 would keep it tight.
That's a fairly old piece, you might want to cock it very slowly to make sure the timing is good. If it is good the cylinder bolt should drop in the cylinder notch and lock up tight.
I was certain that the ejector rod was the cause.
One of my 19-3s has had its rod unscrew a couple times in the past. I think I put a bit of blue locktite on it the last time, and it's been staying put since then.

I have one of these; works fine. However you can get by with a small vise and leather pads to hold the tip of the rod while you tighten it (LH threads!) No need to over do it--just snug it up. I used to put some blue loc-tite on those threads and that was the end of that problem.
The funky mystery tool...its cool.
Don't waste your money on the funky mystery tool. A pair of pliers and a leather glove will solve this issue handily. A little blue Loctite can help as well.
Light night I used pliers (with an old rag covering as to not scratch) and loctite- seemed to fix the issue. After letting it cure for 15-20 min I repeatedly opened and closed the cylinder- ejection rod is still tight.
Looking forward to taking her to the range, her trigger is even better than my 581!
