Bad ammo is my guess based on the several other guns i examined with similar results. You can probably search the forum and see them.
I received my new stripped upper today and I was reassembling the pieces and I found out that a round did go off out of battery. There is a concave dimple inside where the lugs are located and the barrel shank has a convex dimple protruding from it. Some crazy stuff must have happened for this to happen. Any ideas?
If a round fires out of battery, wouldn't it become a blowback? I would think the case, bolt etc would get pushed back. Even if lugs only partially engaged that should be able to be seen. Unless it's a slamfire, it's impossible on most ars.
I'm just going to say it was a horrible case of bad luck and move on. I found the same barrel I had before on clearance at Midway.
This was posted either here or elsewhere and I saved it for training value.

So as the bolt starts to retract the firing pin moves rearward?
