I've gotten results like that with an upper and it was RMA'd after some back and forth. Nowadays I just usually stick to brands I've had good luck with accuracy-wise.

I disassembled it, and the torque on the barrel nut was solid but has to be about 70ft lbs to get the teeth to line up for the gas tube. The muzzle diameter is .224 - exactly the same as it is on another AR I have sitting next to it, so it is not constricted.
The manufacturer has agreed to look it over, but I am going to shoot it one more time this weekend with no muzzle device and several different types of ammo to see if maybe reassembling it has fixed any gremlins that were hiding in the shadows. If it doesn't improve, I'm dropping it in the mail. I will take MM's advice to shoot it side by side with a rifle by the same manufacturer with the same ammo and the same optic.
I would be tempted to slug the barrel and check the bore diameter.
For the record, what is the barrel twist rate?
