I am no expert but it is hard for me to believe that the BAD lever could put enough torque on the bolt catch to snap it off. For one thing you are using one finger not your hand on a large lever, and the amount of movement either way is limited. From the picture it looks like a faulty cast part.
I have a Phase 5 Tactical bolt release, before the BAD was available, on my M4 and I have not been gentle with it at all. Looked at my bolt release, RRA, and it shows no signs of cracks or bending.

Well, I'll be the first to say that things break. I've got literally thousands of rounds down range through several rifles all equipted with BAD's to include hours of dry pratice. Travis and Chris have well over a hundred thousand rounds with BAD's. We've sold tens of thousands of BAD levers and this is the first I've heard of this. Shit happens, bad bolt catches are out there. Not much else I can say. Does the BAD lever cause more stress on the BC, sure. Has it caused an issue before Cold Dead Hands? Not that I've heard of. IMO remove, replace and move on.

The bolt catch is a cast piece of metal and I would bet there was a flaw in it. Call LMT.
