When I was in Iraq from 07-09 we had 500 BM carbines. Not one carrier key was staked until I started the process. Numerous keys were loose, many did not have the required torque applied and some of them worked loose when they were being shot at the range. The weapons were purchased new in 2004 if I recall correctly.
So though it took a while for it to happen, it did happen. The question you have to ask is why such a simple process like this ignored and why isn't it done? I am still waiting for some intelligent response to this question.
