Fixed it.
Found a regular carbine buffer I had from a previous AR, cleaned the shit out of the chamber and bolt, re-lubed it, took it to the range and it worked flawlessly with the .223 American Eagle ammo.
IMO, the problem(s) was what several of you mentioned:
1-Running a H-buffer on a mid-length gas system
2-Not giving the chamber / bolt a proper cleaning after getting a brand new rifle
