I have Questions about Forum 1 paper work.

(A) Where do I go to find the "Certificate of Compliance" to fillout? I got the finger print cards and Forum 1 in the Mail from the ATF. (B) What do I put in the blank for the question for "State why you intend to make firearm"? (C) Do I get Orion to ingrave my whole name or just use my Intials? Like this example ( (John Doe Smith Jr.) or ( J.D. Smith Jr.) or J.D.S.Jr. ?

A) They are also on the ATF's website
B) "To enhance my collection"
C) I would do "J Smith Jr"

Regarding that question, I put "Any and all lawful purposes." I have been told that just about anything that doesn't sound completely insane (like "I'm going postal" or "I'm going to get back at all the people who were mean to me in high school") is acceptable.

I ordered all the paperwork from ARF and only took about 5-7 working days. I still would like to know from the guys who have all ready got their SBR Tax Stamp. My problem is I am not sure how to get my first and middle and last name & Jr. written right on my soon to be SBR Lower. Should I get Orion Ingraving to do, to Ingrave on my LMT Lower,Example "John Doe Smith Jr. or J.D. Smth Jr." on my LMT Lower?

As M4arc said I'd also use:

J Smith Jr

For reason I've used:

collection and investment
all lawful purposes

Without looking it up, the name on the SBR engraving needs to be fully recognizable as what ever name is listed on the Form 1.

Both of your examples work...

However you also need to include full city name and state abbreviation.

So this would work:

John Q. Public III
Las Vegas, NV

But this probably wouldn't meet the spirit of the code:

JQ Public
LV, NV

Certainly, leaving out a letter of Smith, to make Smth would be bad.

JD Smith Jr (probably would only work if you sign legal docs as JD)
John Smith Jr
John D. Smith Jr
John Doe Smith Jr

FIRSTNAME LASTNAME

CITY, STATE

On my personal SBR (before I created a RLT) I had engraved the below and then added in on the line for 'additional description':

I had my name engraved exactly as it appears (first name, middle initial, last name) on the Form 1, just to avoid any confusion. Below that it says "Louisville, KY" and nothing else.
