Primers Falling Out

what load are you using? I've had these from just worn out brass, from improper pocket sizing and from hot barrel cook-off. Overpressure usually leads me to see light piercing of primers before blow-out of primers but I'm cautious about load work-up.

I'm reloading an assortment of brass with WSR primers and Winchester 748 powder @ ~24.5 grains.
