Take the upper off the lower, then strip the bolt from the carrier and install the charging handle and carrier in the upper.
Hold the carrier in place fully forward then point the upper straight up with muzzle at the top.
    Release the carrier.

If things are getting too bad, the carrier will knock the CH loose from the latch notch upon impact.
That means something must be done as it could also occur during firing
Might just be a soft push, might stuff it through your face. Nothing I'd want to bet on though.

Try it with you old handle first (based on your description it will likely fail) then try it with the replacement.
If the replacement pops the CH loose, do something about the upper ASAP regardless of a seemingly inocuous appearance.
What you just said.....makes no sense.

The carrier will never come into contact with the rear of the CH when the rifle is assembled. If it does it generally snaps the latch it two. What keeps this from happening is the buffer. The buffer will bottom out in the receiver extension before the carrier key comes into contact with the lower or CH, generally there is a 1/4" gap between the lower and the key when the buffer bottoms out.

    You're right.
The side of the tube or top shoulders of the square section could never apply a force on the bottom or inside of a CH.

The OP describes the CH popping loose during firing in the very first post.
Something had to be making it move.

My post above was strictly to test the condition of the receiver portion of the latch system not determine what was doing the pushing.
I've seen failed ones in the past, even had one here of my own in the past year, and can assure you the CH does move if the notch is messed up. Just a matter of how softly or violently.

It can, but it wont "stuff it through your face" like you mention. At most it would make it loose and give you a little tap on the nose, you would barely notice it.

That said, it would be a rare case, the bolt carrier has very limited movement and it would be rare for it to really grab the CH and really spit it at you, if it happens I suggestthe lottery.

Well its all lottery anyway isn't it? LOL

How do you think them poor buggers who have the chamber blow apart feel and thats (probably) a less frequent occurance than a worn receiver/CH latch/CH latch rollpin issue whether the CH moves or not.

ETA: didn't mean for post #22 to read as pissy as it seems to appear upon re-re-reading it BTW. My hunt-and-peck doesn't always translate my charming intellect, beutiful articulation, gift for nuanced intriguing conversation, and general pleasant to be around-ness
