I would think that most people measure center to center and use the longest distance as the group size due to distortion/tare in the holes of the paper.
There are only three types: Trained, Un-trained or JAFO.
Which one are you?
I use dial calipers and measure from the edge of the farthest two holes. Then subtract the bullet diameter. This will give you your center-to-center group size.
5pins method is the most commonly accepted for measuring Extreme Spread. If your measuring device is too short or you don't have all of your bullet holes completely on the paper, you can modify it to measure from inside edge to inside edge of the two farthest shots, and add caliber.
Calculating Mean Radius is a lot more complicated, which is why most people stick to Extreme Spread.
I shoot and hand it to a wicked smart guy!~

Someone should make caliber specific templates. Ned are you listening?
I have always refered to MOA as,
1) the way to measure one click on a scope to equal how far the "Cross Hair" moved at 100YD. IE:1/4 MOA it would move 1/4" at 100 YD, 1/8 MOA= 1/8" at 100 yd.
2) The size of a red/green dot covers at 100Yd. 3 MOA covers 3" at 100 Yd, and 5 MOA covers 5" at 100 Yd.
We always measured groups between the two farthest holes with a straight line, from the inside of one farthest to the outside of the other farthest hole. Wich gives a center to center, farthest for group size, max.
Just my $.02
