My Dept of State issued BM in Iraq was not staked at all and came loose in short order. My company armorer ordered a MOACKS out of his pocket and made it his business (after he was shown the problem) to stake every rifle he could get his hands on.
It just takes a few extra minutes to do the job right, but I guess that BM and others are more concerned with the pennies they save and not the lives they risk...
We have six brand new RRA's, and each eas insufficiently staked. I recognized and fixed it with Ned's MOACKS, so it did not cause a problem, but I was surprised that RRA sent them this way.

I've had it happen on an older AR. PWA lower with an unidentified upper.
3 LMT carrier keys came loose on me,No damage.
I see it often I now pack my PMOAKS and wrenches when I go to the local range as someones brand new AR almost always goes down.
Normal for them, too.
Curiously, it's also normal in their premium/flagship/bench-built Government Model as well.
Beware- Tighter tolerances found in some Rock Rivers has led to binding of the carrier in the upper receiver if the staking displaces any material to the side. A swipe of the file corrects it, but it's worth noting. Same carriers dropped into other brands showed no such anomaly.

I've never had a problem as all of mine were staked properly. Except for my lefty Stag which I sent to Grant to stake correctly before I even fired it. No problems in the past with Colt or LMT (the only other BCG's I've had experience with) but they tend to be properly staked.
My 11.5" Bushie upper became a single shot rifle after only 200 rounds. I wish I knew then, what I know now.
