    OP, so your saying that the buffer spring was swapped out and problem went away? Although I have never personally seen an out of spec buffer spring , I guess I wouldn't rule it out, especially if is acquired from a low quality supplier. Just doesn't seem like that's what was wrong.
    Hammer spring installed correctly. I think that one of the earlier members that replied got it right. H2 buffer with crappy spring caused carrier bounce which caused failures on fast double taps. If I fired with a slight pause in between I had no problems. Since replacing the spring and buffer the problems went away.

    Which buffer are you now using ? Another vote for Sprinco. I think I use either the blue or white spring. Have you tried the Vltor A5 kit?

    Still does not make any sense.

    If it only happened on the second shot of a double tap, this is telling me that there is a slight delay with the bolt going into full battery - which does point to buffer spring. One would think you would feel that problem, unless you are anticipating the shot and putting extra torque/flex on the upper causing something to bind.

    Lube, bolt quality, cam pin...a few other things to consider, but the spring did fix it.

    Yes spring fixed it. Bolt is BCM. Buffer BCM h2. Lube is tw25. I will get a BCM spring to match the BCM bcg to use on my next build. I really had no idea a spring could cause such an issue. The new ST2 spring buffer combo is working.

    Are there any visual differences between the ATI and your replacement? Other that just complete lack of QC control on ATI's supplier, just don't see how a spring could be that far out of spec in terms of spring rate.

    ATI spring is about two coils longer and the spring tension is softer.

    Sounds like a carbine spring got stretched out fatiguing it and reducing its spring rate OR a rifle length (A2 stock/buffer length) got "made" to fit....
