Get this one and put it at the small of your back. Save on belt space and you have a bit of extra lumbar support, plus you can grab it from either side if you are injured. For reference, I bought several ifaks that were oriented vertically before grabbing this one and I would never go away from this design.

A couple years ago I started moving towards IFAKs that are centered on the lower back-- it's easy access by either hand if something were to happen, out of the way, and frees up real-estate elsewhere. I believe Sotech was one of the first to start the centered-back IFAK trend.
I've used the LBT-9022B pouch quite a bit and really like it for how low-profile it is. If you want to test it out as a proof-of-concept without worrying about spending big money, their off-shore subsidiary, LBX Tactical, makes that exact same pouch at a much lower cost.

I don't have anything to do with this company other than ordering from them a few times with no issues. I appreciated that they swapped out a belt that i found too large for a size smaller no issues.
IMHO, this is THE BEST IFAK I've found for the size. You will not find a better thought out or better packaged IFAK.

I not aware of anything small enough to carry on your person in normal clothes unless you have big cargo pockets.
