Knowing BCM customer service, they'll probably fix it. If not, then just crank 'em back down. It's actually up for debate whether staking does much good or not. It certainly doesn't hurt, except that it's permanent, but as you have experienced it's entirely possible for even properly staked gas keys to come loose.
MOACKS - 85.00. Cost effective and cool if you are going through weapons or are building bolt carriers. Not so cost effective, if you are just taking care of your weapons.
How many BCG's are you going to buy with unstaked gas keys?
If I did not have access to one, I'd move the metal myself. I'd go to a local smith that has one and see how much he'd charge. Take it apart, inspect, get new allen heads, install and torque, walk in and ask how much to stake this?

Does anyone know if any manufacturer or builder ever pinned the gas key bolts?

I would be worried that one of the bolts is broken. The staking holds the top in place, but it's separated from the part in the carrier.
