There are two little springs in the side of the mount with the lever. If you take that lever off to adjust your scope rings and do not reinsert the springs, that might cause an issue. (that's if you choose to use your own tools to adjust the torque on the screws) It's easy to miss, they just pop out as soon as you take the tensioner lever component off.
That would not be the issue with his mount that you see in the video. All the newer mounts for a few years now have came with the mounting bolts at the top side of the top rings.
OP, I would definitely take it apart and take a look at that spring for the locking lever that the above poster is talking about though. He could be onto something there?
