You're right! I also didn't notice that the rifle was upside down in the photos.

Maybe it's just the photo that's upside down.

I'll say it one last time- all three malfunctions are due to a problem with the extractor. The extractor loses control of the case before the mouth of the case clears the ejection port. Without the extractor controlling the case, the ejector cannot eject the empty. The bolt then strips a new round from the mag resulting in a bolt over when the fresh case jams against the unejected empty.

Short stroking whether it's due to ammo, buffers, springs or gas leaks won't cause this kind of malfunction. The empty will be ejected before the bolt travels rearward enough to be able to strip a fresh round from the mag. That means it's physically impossible for the bolt to strip a round from the mag unless the extractor loses control of the empty, preventing proper ejection.

The ejector of the AR uses a spring that constantly puts pressure on the case so long as it's held in place against it by the extractor. As soon as the mouth of the case clears the forward edge of the ejection port, that spring flings the case out. This happens at a point before the bolt can catch the rim of the next round. Therefore, if there is an empty and a round stripped from the mag jammed into the action, it's due to the extractor failing to hold the spent case.

Could it be a bad mag? No. A bad mag doesn't keep an empty case from ejecting. Could it be caused by the Spikes buffer? No. The Spikes buffer can only change carrier speed and a faster carrier speed can only cause this kind of malfunction if there is a problem with the extractor

^ I get it, I've been searching left and right on this and this seems to be it. But before I went tearing the extractor apart I emailed BCM to make sure they didn't need to first see it or something before I possibly get the blame. I don't know what BCM's warranty or guarantee policy is because I've NEVER had a problem with there products before! I'm guessing I'll get a reply some time next week. I'm heading out to the range as soon as my rear sight is delivered today and I'm going to try a different bolt and so forth. Thanks everyone for their input and I'll make sure to follow up with my results, if anyone cares

BCMNick, I didn't mean to sound like I was getting after you. I was a bit cranky yesterday when the meds started wearing off and the pain kicked in and let myself be frustrated with posts I felt were muddying the waters

Brand New BCM rifle --I MODIFIED-- *Failure To Eject*

I wouldn't consider any of the advice given here as "muddying the waters". Starting out with an unmodified rifle as received by BCM actually sounds like good advice & was even suggested by an SME.
Maybe it's best not to post when under the influence of narcotics.

Ok, sorry for not updating, I'm a very busy man.

Went to the range Saturday evening with both my faulty rifle, well used BCM rifle and my other never fired BCM rifle. I first tried swapping out buffers and buffer springs, nothing changed (I didn't think that was ever the problem to begin with as I used the spikes heavy buffer and JP polished springs in all of my rifles) Then I swapped out my fault BCG with the Brand new BCG and no problems what-so-ever! I even went as far as just swapping out the faulty bolt with the new bolt on the original BCG just to make sure there wasn't a gas issue or anything else and it still worked. BCM has since replied to my request and sent me a shipping lable to ship the entire upper back to them. But I just received another email from them asking if I would rather have them ship a new extractor to me today instead of shipping the entire upper to have it repaired. So I guess they also assume it's a faulty extractor.

Would it be possible to disassemble the bolt and show some pics of the extractor and the spring?
