Can you provide some details? Like who makes this weapon and or parts? With that many rounds down the barrel, I am going to rule out the typical chamber issue which is that it is out of spec (as in made wrong).

My gut tells me that you have a gassing issue and I suspect it is the block being installed wrong. The other possibility is that the gas tube is worn.

Whole upper with barrel is made by StagArms. Handguard, gas block is provided by different vendors. Bcg is also Stag. Adjustable gas block was SLR (iirc). Gas pipe is working more-less. I can blow an air throught it.

Ok, problem solved. I don't know what helped.
    What i did:
    1. Switched back to non adjustable gas block.
    2. Reassembled upper (removed barrel from upper, cleaned and install it again).
    3. Got new gas tube (fortunetally friend of mine had a spare one).

I've shot about 150 after that without issues since that.

Anyway - thank you guys for help.

Your gas block looks misaligned (there should be a small gap between the shoulder and the block) and your gas tube looks (based on crappy pics) worn where it contacts the carrier key.
