But one form will do you for one year for each site.

Say you were going from Princeton, WV to the Triangle Sportsmans Club in Bluefield, Va for matches, you could make one form cover you for an entire year.

Triangle Sportsmans Club happens to be exactly where I would be taking it. I guess it would be worth the hassle if the form will last a year. Can I send in the form to transport across state lines at the same time that I send in the paperwork to possess the SBR of do I have to send in the paperwork to transport the rifle afterwards?

I'd wait until you have the approved Form 1 in your hot little hands. They might try to process the 5320 before they get to the Form 1 and this could muck up their process and cause a delay.

FWIW, my last 5320 took less than 2 weeks to process.

    Exclamation

Uh, guys, you know you can get same day service on your 5320.20's for short-notice travel if you FAX THEM IN.

Call ATF NFA office in West Virginia. Have the S/N of your NFA item handy, they will then refer you to your inspector.

On more than one occasion, I've included suppressors on a 5320.20, even though that is not required. ATFE will approve them ... it's a nice piece of paper to have if you get pulled over in an anti-gun jurisidiction (NY, NJ for example) where the LEO might not be 100% versed on federal gun laws.

Damn, I must be Psychic or Psycho or something like that

You need to get the SBR approved first.
