Bent firing pin. It was a factory option with Pac West (PWA) BCG's, back in the day. In fact, I never came across a PWA that DIDN'T have the bent firing pin/doubling option.

My brothers would double until we replace the disconnecter and its spring.

had it happen over the years due to bump-firing, too-light springs, or disconnecter issues (including one that had been incorrectly fitted)
