Ever seen indentations like these before? These are out of a brand new upper, and are Lake City 2014 stamped. Ejection was to 3 o’clock and everything else seemed normal and functioned as expected.

Looks to me like the brass is smacking that front corner pretty hard. I don't have one of those new uppers, but is that corner sitting further out from the dust cover than a normal upper?

Record the ejection in slowmotion and see what is going on.
Agreed on the sights being a possible, if not likely, culprit. Try removing the rear buis and see if anything changes.
