Form 20 help

I am moving to a new state, filling out a form 20. I just searched ATF form 5320.20 and am using the ATF's pdf. Part 1, 1-6 are done. Box 7 asks transporting from? Is this my current address? 8 is my new address, 9 is private vehicle in my case. I'm not leaving the country so no 10-12. 13 is my signature and I can email the form back to the ATF?
