    It's not uncommon to get a gouge in the anodized non contact areas in the upper from this, it may not be pretty, but it'll function fine. This condition was unfortunately common in the early 90's when cheap Chinese .223 was popular for some. The only times I've seen severe damage from primers is when a previously round's primer gets caught in the BCG cam path preventing unlocking of the bolt.
    What severe damage does this malfunction cause?

    Likely what ever method the user tried to fix the problem with if he did not know what to do or what the cause was. I saw one person pry with a screwdriver on the edge of the carrier against the front of the ejection port until it squashed the primer enough to unlock. Gouged the upper some doing it. That was after they had butt stroked it pulling on the charging handle a bunch trying to open it.
