Theres not many. CMT has some, FCD has a slight variation that CMT produces for them. I just got an SLR Ion because as much as I like FCD, I prefer 7-sided MLOK rails. The SLR seems solid and is very well machined.

Rails that are nominally 12, including the poorly named DDM4 12.0, are usually ~12.5 and will not play nice with 12.5 barrels. The longest rails that work well are:
Larue Quad or LAT 11
KAC URX4 10.75
BCM QRF10
SLR 11.7
Triarc Wedgelock 11.5
FCD/CMT RHF 11.5
Troy Battlerail 10.75
Midwest 11.5
G Mk8/16 10.5
DD M4/MFR/MFR XL 10
Aero 10.7
My preference is Larue or KAC. All of the above 10.0 and 11.0 rails are slightly longer than advertised. Some have built in swivels, some dont. Most MLok rails are skinny as ****.

The best Ive seen, and what Im doing is a 13.5 Geissele mk16 cut down to ~11.75
I know this is not what youre asking. But when I went in search of what you are, this was my conclusion.
LaRue makes an 11" MLok rail... Pretty good deal at $135 I think.

Here's the FCD rail on a 12.5 barrel that I just assembled. The flash hider is not timed yet, just threaded on.
