When I received my digital stamp for my form one, there was a blank sheet for leo sign off and photo ect. Since I did it as a trust do I need to hang onto that part?

It's the back for the form, to go with the other side. You probably don't need it as your control number is on the front, I think it would be safe to assume that all of us print the front with the back.

FWIW...

I print a 2-sided copy and stick it with my paper stamps, to serve as an original in case we get hit by an EMP. But, for the copies on my phone, it's just page 1, since it has the stamp/Approved box/examiner's sig. (The control number doesn't really mean much...I actually have one Approved eForm that doesn't have one.

The copies I print to keep in a binder are printed 2-side with just page 1 of two different forms per sheet of paper. So, my current 30+ stamps are only on 16 pieces of paper. I've never been asked for them, but do use them to explain the process to people occasionally at the range.
