He attached the holster using the inner belt.. This setup allows for either usage of MOLLE or belt connection, which is why I like this belt.

I've been wearing a Brokos belt everyday for the last six months thanks to the untimely death of OBL (suck it!) and the generosity of SKD. I'm happy to say that it has held up well and remains a kick ass piece of gear. Daily wear can usually tear up nylon gear but this thing has held up to use and abuse. I wear a Bianchi duty belt inside with a riggers belt to hold up the britches. The Brokos supports a Blackhawk kydex double pistol mag holder, a Serpa drop holster, drop leg gas mask pouch, Vtac molle handcuff pouch, and a Vtac double molle pistol mag pouch for OC and surefire 6PX. All that shit and this thing doesn't show anything for the worse. I'm pretty sure my 4 year old duty belt has had it though. Time for one of those nifty Cobra gun belts now!

I am really liking this setup! To help with sizing: I'm 6'1" 170-75 32" -33" waist and a large Cobra and large Brokos works perfect.
I love the modularity and how easy it is to swap gear around using the Cobra belt. Much easier than having to use Molly or other attachment methods so you can run it slick in short order. The belt is also great in dispersing the weight which really helps my back.
Note: The blade and RCS Moduloader are not worn on the Brokos belt.
Over two coats.
