I think the guy had a squib and fired another round behind it. The empty coming out of the chamber when he was changing mags was a 'clue' that something wasn't quite right.

I agree. There was a projectile in the chamber. Each time he cycled the action (in that convulted manner of his...) he drove it deeper into the barrel until eventually he was able to seat the next round.

That, and some of the new to reloading stories I hear... That guy in the video claims "bad primer" several times and the other guy said "he didn't load these."
Lesson well learned, I hope. Talk about "recoil flinch" from now on...
