Looking at your pics the reason it didn't fire is because the bolt is not in battery. (all the way forward) Nothing scary about that at all.

Glad you got it apart. I am betting on the blown primer being the cause. I have seen that a few times. Darn things can get caught in the charging handle or barrel recess.
Buddy of mine had this happen at thunder ranch after the SURGE. Clint Smith ripped out his forward assist with a pair of channel locks. Problome solved for him at least. It was a AR him and his did built so I dont know what parts where used.
Just for curiosity sake, what ammo were you shooting?
Sounds like a headspace issue with the reload to me.
Tight-ish or heavily foulded chamber combined with a shoulder that didn't get set back far enough would cause this type of failure.

I am aware of a fatal accident that occurred under similar circumstances. A person had a live round stuck in the chamber and tried to pound it out with a cleaning rod. The cartridge fired, he was not seriously hurt but his wife who was holding the rifle by the butt stock was killed when she was struck in the abdomen by the case. You can determine if you are dealing with a live cartridge or empty brass by gauging with a cleaning rod.
As soon as I read your post I thought it myself “ I bet he is shooting reloads “. This is a common problem with handloads that have not been sized properly.
