You can determine if you are dealing with a live cartridge or empty brass by gauging with a cleaning rod.
Wow, killed by the empty case hitting her stomach when ejected from the rifle? No offense but I find that hard to believe. I have been hit just about everywhere with ejected cases including the face and while they can burn I find it hard to believe they can kill. Maybe I am missing something but this just seems very unlikely.

The only way I can think of an ejected case being deadly is if it exploded and she was hit by shrapnel.

Luckily I don't have many left. I bought 1000 and shot probably 700 of them with no problems. Will give the rest back to the friend who sold them to me.

Think I will pick up a 1,000 rds of the PMC X-TAC off aim surplus.

For those of you who won't use reloads other than your own, what about some of the commercially available reloads such a Georgia Arms? Will you use those? Doesn't make much sense to buy them right now (~$.35/rd for canned heat), but I've used it in the past with no problems.

My guns have rarely seen any reload I didn't assemble myself, and that's all some of them have ever seen.

Some reloads from a mfg. I've used and would trust are from these guys:

BHA reman. I trust as well.

I've never tried any GA "canned heat", but I've heard enough internet chatter (which may be true or false) that I'd rather shoot Hornady's steel case training ammo than GA reloads.

    Happened to me

It happened to me and it wound up being that the Firing pin retaining pin broke and lodged between the Bolt Carrier and the rails of the upper. Took some Monkeying around but we finally diagnosed it.

    extractor probably

I also had that same problem happen to me with a 7.62x39 AR I had a while back. The problem was the extractor didn't have the same radius as the rim and would just tear through the brass instead of slipping over it during the chambering action. I had to use a round file to get the right radius, which fixed it. Check the cases to see if theres any nicks or tears in the rim....sounds like the prob. Good luck!

    Glad I found this thread:

    Range trip today, similar problem:

Total round count of the rifle is approximately 980-995 rounds (last 500 or so being either the 5.56 TAP or the 55gr training ammo also by Hornady)

This is a regular BCM 16" midlength upper with a gunfighter charging handle and an LMT BCG, all of which was new when I put the upper together if that helps

First magazine of the day, before the malfunction, I fired 9-10 rounds of the Hornady steel cased 55gr training ammo.

The actual malfunction: Was firing on a target, I pulled the trigger, nothing, Tried to tap rack, couldn't operate the charging handle. Took the rifle back to the bench, opened up the rifle to find both the carrier and charging handle are stuck forward, I can get maybe a couple millimeters of rearward movement by pulling the handle.

In addition, looking with a flashlight, the problem is the lugs aren't lined up for me to tap the BCG back like the original poster was able to do, if I try to tap the BCG back, the bolt would be stopped by the lugs catching on the barrel extension.

Not sure if there is a live round in the chamber or not.

    Any advice?

    Thanks in advance.

How much overlap with the lugs is there? My guess is that if you were to tap it (Carrier, not bolt) out, the bolt would complete it's rotation and unlock.

I had similiar issues with a new LMT and some reloads I had for a bolt gun. While they cycled fine in the Bolt Action every 50 rounds or so I would have one stick in the LMT as you have mentioned here. Usually the mortaring trick was sufficent to extract....but one occasion required more drastic action.
Carefully checking case webs with a Micrometer showed that my full length sizer wasn't properly sizing the casing around the web area.
I now use a Small Based Sizer Die for anything that will be ran in the AR.

Here is a pic I took, sorry its not great, but it looks like they overlap almost completely. So is there a good way to make sure tapping the BCG through the ejection port will cam the bolt so it unlocks? I can barely see what I think is the extrator pin. Do you think I could use some sort of tool to poke in there and lever it so the bolt turns if tapping doesn't work?
